Citizenship Forms

The Muscogee (Creek) Nation Citizenship Card is issued by the Citizenship Board Office.

This card includes a roll number as well as a photo of the Citizen, the Creek Blood quantum only, name, date of birth, and other additional information.

Enhanced Tribal Card (ETC) mentioned on the Universal Citizenship Form is only to request more information about the ETC or to schedule an appointment.

ETC is appointment based and requires an In-Person process.
DO NOT SEND ANY MONEY REGARDING THE ETC FEE.

If applying for a replacement ETC through the mail-in process, please contact the Citizenship office to make sure no other documents are needed.

Relinquishment Form

This form is to relinquish your Citizenship Status from the Muscogee (Creek) Nation.
As of August 23, 2001, a relinquishment from the Muscogee (Creek) Nation of a Citizen 18 years or older, is an absolute and irrevocable resignation.

Reinstatement Form

This form is to reinstate your Citizenship Status to the Muscogee (Creek) Nation.
Additional documentation may be required, please contact the Citizenship Board Office.

True Copy of Identification Form

This form is now needed for any identification (being sent by mail) that appears on the list of Identifications attached to this form.

The Certificate of Degree of Indian or Alaska Native Blood Card is issued by the Bureau of Indian Affairs.

This card includes name, date of birth, blood quantum, and tribe. (This card may display up to all the Five-Civilized Tribes with accumulative blood quantum).

Notice of Other Tribal Enrollment Form

This form is to be completed if the applicant is an enrolled member with another federally recognized tribe but still wants to go through the citizenship enrollment process. The applicant will need to be aware that once the enrollment process gets to a certain point, the applicant will then need to relinquish from the tribe that is listed on the form.
